NHS Guildford and Waverley Clinical Commissioning Group (CCG) has announced it had selected the partnership for the nine-year contract following a “competitive, fair and robust” re-procurement process. Community health care services provide a wide range of care outside hospital, from supporting patients to manage long-term conditions, to treating those who are seriously ill with more complex needs.
It will see teams of nurses and therapists from the Guildford hospital working closely with other healthcare professionals, including GPs and social workers, to deliver care usually in people’s homes.
The service also includes local community hospital services to provide care closer to people’s homes.
The partnership aims to work more closely with Surrey and Borders Partnership Trust in the delivery of mental health and plans to integrate services more with Surrey County Council and adult social care teams.
Planned improvements include the implementation of a single patient care IT system.
